November, 2007 -- This is our new form which allows people to apply on-line. Although we have thoroughly tested it, about 2 or 3 percent of
the people applying have problems. If that happens to you, please copy any error message you receive and then email it to James Mitchell at jmitchell@kensingtonllc.com and please include your name and telephone numbers. He will call you
quickly to try to figure out what is going wrong.
We have been hosting parties in Boston since November 2002. Starting in November 2007, we will also be giving parties
in Los Angeles. Do you want to receive invitations for parties and events in:
Boston
Los Angeles
(If you are bi-coastal (i.e., you spend time in both LA and Boston), you are welcome to choose both cities,
but this is uncommon.)
Name
Full proper name (including any middle
initials or name). Please list your first name first, then your middle initial
or name, and then your last name — e.g.,
John S. Smith, Hillary Rodham Clinton, E. Philio Wigglesworth.
First name (as you want it to appear on your name
tag)
Last name (as you want it to appear on your name tag)
Salutation / first name (how you wish to be addressed, what you want people to call
you — e.g., if your first name is "Elizabeth," you may prefer to be called "Betsy")
Sex
(if you are transgendered, write to us and we will figure out how to classify you)
Password
Your password must be at least 6
characters or numbers long. Case does not matter. Whatever you type in (all
upper case, all lower case, or mixed), we will store your password as lower
case, in order to eliminate confusion.

E-Mail Addresses
Primary E-Mail Address
Please re-enter your primary e-mail address
Please provide us with the e-mail address you wish
party invitations to be sent to. This should be an address you check on a
regular basis. In addition, this should be a "permanent" address — i.e., an
address that you will have for a long time. If you're employed by a company and
you might not be working for them two years from now, then wouldn't it make more
sense to have invitations sent to and you have an alternative e-mail address
(such as Hotmail, gmail or Yahoo), it would probably make more sense to provide
us with your alternative e-mail address.
If possible, please do not
provide us with an AOL address. AOL regularly flags a lot of e-mail as spam,
even when it is not spam (these are called "false positives"). When we then try
to call them about this problem, the technical support is literally the worst in
the world.
Other E-Mail Addresses —
Please provide all of your other e-mail addresses, so that we can contact
you in case your primary e-mail address bounces.
Telephone Numbers
We need to have all of your telephone
numbers — home, work and mobile. We ask this for two reasons. First, if all
of your e-mail addresses go bad, my secretary needs to be able to contact you to
find out your current e-mail address. Second, on rare occasions we have to
cancel events at the last minute. Under such circumstances, in addition to
sending you an e-mail, my secretary will call you.
If you don't have a work telephone number (you're a
student, for example), then please check the applicable box. You should do so
only if you truly don't have a work telephone. If you say that you don't, but
you work for a company and you do, my secretary will simply deactivate your
membership and you'll no longer receive invitations.
For each phone number, you should also specify whether
we should telephone you at a particular telephone number. Choices are:
- 1 — Preferred
We should try this number first
- 2 — Fine to Call
Calling this number is also
fine, but try the preferred number first.
- 3 — Second choice
We should try the preferred
number first.
- 4 — You prefer us not to call you at this
number
We won't call this number unless we are unable to reach you at the
other numbers.
- 5 — Don't call
We will not call this number
unless (i) it is an emergency (i.e., you're attending a party that evening and
it just got cancelled) or (ii) every other number is not working.
If you are indifferent as to whether we call you at
one number or another, then tag them both as 1 — Preferred. If you
have a particular telephone number but do not wish us to contact you at that
number, then you should supply that number and then flag it as 5 — Dont'
Call.

Residence
Which city and state do you live in?
What is the zip code of your primary residence?
What is your profession? Please provide as much detail as possible, ideally
several sentences. If you are a student, please list where you go to school, what you are studying, and
when you expect to graduate.
Marital status
Relationship status
If you have children, please list their names and year of birth
Education
Please list all schools attended, degrees obtained,
and your major/concentration, and when you graduated.
Year of Birth
We ask that you provide us your year of birth. We ask
this because going forward, many of the parties we will host will be age
specific. (The large cocktail parties will always be oriented to all ages,
however.) Please note that we do not need your exact year of birth, you can
approximate — if you were born in 1980, you could list 1978 or 1982, and
that would be close enough. After you become a member, you can change your year
of birth if you wish to record a different year. For further information, please
read the section in our "Frequently Asked Questions" on year of birth.
Please note that we do not
provide your year of birth to anyone, even our facilitators. We use it solely in
our database for sorting which individuals are invited to which parties.
What is your year of birth (year only)?
In addition, we ask that you tell
us the age range of people you ideally would like to socialize with. This
information is used primarily for dating purposes — e.g., if we were
inviting you to a speed dating party, and you want to meet people who are
between 25 and 35 years old, we would use that information in determining which
events to invite you to. You do not have to provide us with this information,
but if you do not, there will be certain events that you will not be invited to.
After you become a member, you can change these age preferences at any time.
What is the minimum age of people you ideally
would like to socialize with (primarily for dating purposes, rather than general
socializing)? Please enter the year of birth for such age. If you are totally,
completely indifferent, enter the year 2050, which means there will be no
bottom limit on the age range.
What is the maximum age of people you ideally
would like to socialize with (primarily for dating purposes, rather than general
socializing)? Please enter the year of birth for such age. If you are totally,
completely indifferent, please enter the year 1900, which means there will be no
upper limit on the age range.
Reports of People Attending the Parties
Before each large and smaller cocktail party, we send
one or more reports to the other people attending. These reports list the name,
descriptive tags, descriptions and any public (but not private) requests to the
facilitators. You can choose to receive none, some or all of these reports. You
can choose as many reports to receive as you would like.

Hobbies
What are your hobbies and interests? What do you do for fun?
Why Are You Interested in Joining?
Why are you interested in being placed on the
invitation list? Please be as specific as possible — e.g., "I want to make
new friends," "I'm looking for additional business contacts," "I want to meet
someone to date"
If one of the reasons you are
joining is for social reasons (as opposed business networking or for dating), it
would be helpful if you were as specific as possible in what you are looking
for — "I just want to meet other women," "I'm looking for a tennis
partner," "I want to meet people who collect stamps."
If you're interested in joining
for business networking purposes, it would be helpful if you were as specific as
possible in what you are looking for — "I'm looking for a new job," "I want
to change careers," "I am a freelance Web designer and I'm always looking for
new clients," "I'm trying to start a company and I want to meet potential
investors," "I'm looking for a business partner," "I'm looking to hire several
employees." If you answer this question, the facilitators may read what you
wrote and then try to introduce you to other people.
If you're interested in joining for dating purposes (at least in part),it would be helpful if you were as specific as possible.
- Tell us what you are looking for in a romantic partner. List everything that matters, from physical characteristics to other characteristics.
- Tell us what you are not looking for in a partner.
- Tell us the age range (minimum and maximum) of the person you are looking for.
- Tell us what kind of relationship you are looking for -- "I'm looking for a casual fling," "I want to fall in love but not necessarily get married," "I'd love to meet a life partner and get married."
- If you're on any on-line dating sites, please tell us which ones and your screen name.
If you tell us this information, our facilitators may try to introduce you to people they think you might be interested in meeting.
Please note that these are social
parties, not singles parties. Lots of married couples and those who are in committed
relationships attend our parties. On the other hand, if you are single and looking, it would make sense to
attend our parties, since many people who attend are single and looking. We sometimes do host singles parties,
but those are different from our regular parties.
Tell us about yourself. Where did you grow up? What should we know about you that is not covered in the other questions on this form?
Internet Dating Sites
You are not required to provide us with this
information. If you do, James and the facilitators may read your profile and try to
think of people in the group you might want to meet. If you were interested, we
would then make an introduction, either by e-mail or at a party.
